The Wisconsin Interscholastic Athletic Association publishes health and athletics resources. Check its current Health & Safety and Publications pages, then confirm any additional requirements with your school.

Ask for the part the school needs

WIAA’s physical information directs schools to collect the Medical Eligibility Form rather than the medical-history pages. An uploaded packet can contain extra pages or be in a different order, so identify the document by its content.

Include school requirements

Your school may also have its own policies, acknowledgments, and program requirements. Tell families which items are required for participation and which are administrative reminders.
